COVID Visitor Self-Screener

The Pasco County School District expects all visitors to complete this pre-entry self-screening tool daily before physically visiting any school campus or district office location.

 

  1. Do you have new or worsening onset of any of the following symptoms?
  • Fever (greater than 100.4) or chills
  • New uncontrolled cough (if you have established chronic allergy or asthma, a change from your regular cough)
  • Shortness of breath or difficulty breathing
  • Fatigue
  • Muscle or body aches
  • Headache
  • New loss of taste or smell
  • Sore throat
  • Congestion or runny nose
  • Nausea or vomiting
  • Diarrhea

YES ___ NO ___

 

If NO, please continue to question number 2.

If YES, you cannot come on campus and are required to:

  • Not return for at least ten days since symptoms first appeared,
    • AND be fever free for at least 24 hours (without taking medication to reduce fever)
    • AND have improvement in other symptoms (cough, shortness of breath).

 

 

  1. Have you recently tested positive for COVID-19 or are waiting for test results due to your own illness?

YES __ NO ___

 

If NO, you may visit school as planned.

If YES, visitors waiting for test results must remain home until results are available.

  • If test result is negative, you can visit the school campus.
  • If test result is positive, you cannot come on campus and are required to:
  • Not return for at least ten days since symptoms first appeared,
    • AND be fever free for at least 24 hours (without taking medication to reduce fever)
    • AND have improvement in other symptoms (cough, shortness of breath).

 

 

The novel coronavirus, known as COVID-19, is an extremely contagious virus, which can cause serious medical conditions, including death.  COVID-19 has been declared a worldwide pandemic by the World Health Organization, and as a result, federal, state, and local governments along with federal and state health agencies recommend social distancing and have, in some circumstances, limited the congregation of people. The District School Board of Pasco County (DSBPC or District) has initiated reasonable, precautionary measures in an effort to reduce the spread of COVID-19.  However, given the extremely contagious nature of COVID-19, the DSBPC cannot guarantee that you will not contract the virus while attending or engaging in school-related and/or extracurricular activities.
