PVMS Learning Option Voluntary Change Request Waitlist

Please complete this form if you would like to be put on a waitlist to request a change in your child's learning option for 2nd semester.

All waitlist requests will be approved by an administrator and a confirmation email will be sent, noting the date your requested voluntary change may begin.

Please note, the learning option you are voluntarily choosing to move to may not be immediately available and you are being placed on a wait list for an available opportunity.

Semester 2 for PVMS students is different due to the IBMYP framework and our semester begins January 15, 2021.

I understand that after submitting this voluntary learning option change request, my child is expected to continue attending classes in their current learning platform until I have received an email confirming that the new learning option has been granted. I understand that my MSOL child will be responsible for all devices and resources that were deployed (device, textbooks, etc) will have to be returned if choosing the Traditional setting (Option 1). If a device or resources are lost, I will be responsible for replacement of any at my cost. I understand that by voluntarily changing our learning option choice, my child's schedule will not reflect the same teachers or classes in the same order. I understand that my voluntary learning option request will be placed on a waitlist and granted if an opening becomes available then notified by email of the new start date. I understand that future changes will not be facilitated and this request, when granted, will be for the entirety of Semester 2 which ends May 26, 2021.
By signing this document, I understand that:

- If voluntarily selecting mySchool Online that I will need to provide my own technology until a device is available.

- If voluntarily selecting Traditional; transportation will be my responsibility until a route can be established, all resources that were deployed for MSOL must be brought back to the Traditional setting and devices returned before a new schedule will be released.

- I understand that this voluntary request is to be placed on a waitlist and that my child will remain in the original learning option until an email has notified me of a new start date and that the schedule will not mirror the original schedule issued at the start of the 2020 school year.

- I understand that this is the last opportunity to change the learning platform. Whatever option I choose will be followed until the end of the school year, May, 26, 2021, if granted.
